• Background and Description of Problem
o Clearly define and provide a comprehensive description of the problem that is clear to the reader.
o Should consider elements such as the nature of the unit , patient population served , the staff , and the type of health care organization in which the problem is occurring.
o Define the management problem needing change & identify a specific change model and how it will be used to effect the needed change
• Causes
o the “who, what, when, where, why, and how” elements of the problem at the clinical agency – Must keep anonymity of agency.
o I performed a survey and made the results into a histogram.. see below “SURVEY” I think it would be good to maybe include that in this section. The nurses ranked what they thought was keeping them from completing the whiteboard.
• Evidence from Literature
o What does the evidence say to validate the problem? What are recommendations to address the problem? Think outside the box.
o Consider that some of your interventions will need to be aimed at motivating and leading change.
o Provide evidence related to how you will lead the required change.
o Scholarly articles no earlier than 2016
• Key Stakeholders
o Identify key stakeholders (who will be affected by the change, why they are stakeholders, how are they affected)
• Facilitators and Barriers
o Identify current and anticipated facilitators and barriers operating in the change situation to promote resolution of the problem.
o Connections between stakeholders, facilitators, and barriers should be explained.
