A definition of the agency the case manager is employed by. What types of services are provided? What is the type of case management that is available? What are the primary responsibilities of the case managers? What clients are typically serviced at this agency? How are services paid for? What is the length of time of the average case management service? What is a typical day of this case manager while at work? 1 – 2 pgs.
Part II – Service Plan

Service Plan – both goals and objectives must be documented in the service plan (1 page) At least three goals with supporting objectives is required. 1- 2 pgs.
