Case Study: overview
Choose a patient whose care you have been involved with and identify TWO key nursing care problems and discuss the nursing care required.
The focus of this care study must reflect the theoretical content from Nursing Care and
Management of the Adult with Chronic Health Care Needs.
For this assessment you are required to write a 3000-word case study that reflect the module learning outcomes. It should be based on a patient you have cared for in your practice placement experience who has a long term/chronic condition. You are required to discuss the nursing assessment and identify 2 patient problems and discuss the associated nursing care. In your discussion you will need to discuss the nurses and MDT role as well how the patient was educated to self-manage their condition. (NB. For this assignment, please ensure no individual or workplace is identifiable. (Confidentiality and anonymity must be maintained)
Throughout the assignment you are required to use contemporary evidence-based literature. By doing this you will demonstrate your understanding of the need to provide contemporary and evidence-based nursing care for people with an identified a long term or chronic condition. (NB: the literature you use should be evidence based and from reputable sources.) The Harvard Referencing system should be used throughout the assignment and in the reference list.
Performance criteria and weighting
Relating to your chosen patient, the care study should include: –
An introduction to the patient and care setting; identify the relevance of your choice in relation to contemporary nursing practice. This should include a brief social and medical overview of the patient and identify any family/significant others (10% 300) approx. words)
Discuss the nursing assessment undertaken. This should include a discussion of the Model and assessment tool(s) used to guide the holistic assessment of this patient. You should also discuss the key nursing skills used during the assessment. (20% 600 approx. words)
Identify from your assessment key patient problems related to the patients long term condition. Identify 2 problem / needs that you have chosen to discuss in more detail (10% 300 approx. words)
Identify and discuss with rationale the nursing interventions provided for the chosen problem; 30% 900 approx. words
Discuss the evaluation of care provided for the chosen problem including identification of the skills/tools used for this evaluation. (10%, 300 approx. words)
Discuss the education the patient requires for self-management of their main health issues. critically discuss the role of specialist nurses and members of the multidisciplinary team (20% 600 approx. words
